You might need to report the coupon history issue with OpenCart directly. However, it might also be due to your coupon settings. Make sure that you the customer MUST be logged in to use the coupon. That way, there is a record of the customer using the coupon (this is a coupon setting).
Trying to explain these coupon limitations is a headache, because it's vague. The best way to look at this is to remember that each coupon has a single identification number. So let's say we're dealing with coupon #300. With your current settings, you've said that coupon#300 can be used an unlimited number of times by ANY customer.
The second setting that you made says that Coupon#300 can be used ONE time by a single customer. You need to limit the usage of the coupon using both settings. These settings are NOT limiting the number coupons, they're limiting only the number of uses - by either the Coupon itself, or by customer.
So...if you want to limit Coupon #300 so that each of your customers can use it only once, you need to make the uses per coupon limited to 1 AND the uses per customer set to 1.
You can start to see how this gets a bit confusing. Hopefully, my explanation helps a little. Just remember that those settings are not there to limit the number of coupons, but to limit usage PER coupon or per customer.
