my question is how can i create a contact us page with labels, textboxes and a button (submit) and after clicking submit i should receive a email message to admin

Hello Generation007,

Check out the plugin for Wordpress call Contact Form 7 - we have a good article on how to use it. I'll add that to the bottom of this reply. This is assuming that you are working on your website which appears to be using Wordpress. Here's the link to our articlle:
