form email attachment problems and php

i found an excellent zip file containing a form with validation that allows someone to send you an email with an attached file through your webpage. I followed the instructions which had me install PEAR "mail" and PEAR "mime". I did that - I do see those files in my dir paths which is home/myusername/php/Mail/mail.php
home/myusername/php/Mail/mime.php

the page - which is in my public directory contains the script where i have those exact 2 lines stating where those files are.

when i run the form it submits the image to my dir but gives massive errors - about 4 of them, all stating that it cannot find mail.php or mime.php - are these paths correct? this php directory is hidden from the public - is this why my webpage in the public dir cannot see it? I tried changing to ../ etc with no luck. anyone tried installing one of these forms? your help would be much appreciated, i am launching a site soon and cannot afford to hire someone for this issue - and i am this close lol - thanks - please contact dillenger2006@hotmail.com thank you, Ken

Hello DillengerX,

I'm sorry you're having trouble setting up your submission form on your website. I tested this out and did not receive any errors on your submission form. I also did not see any errors in the cPanel error log or your PHP error log. Can you please advise the exact errors you are seeing and where you see those errors? Also, please feel free to let us know more details about the form you are using so that we can test this out and help you implement this on your website.
